Every month, I write an article featuring one of my passions: health and longevity, or “health-vity”: practical things we can do to increase not only how long we live, but also our years of good health.
Recently, at the recommendation of a physician friend, I finished Back in Control by spine surgeon Dr. David Hanscom. While the book focuses primarily on chronic back pain, what stayed with me was not just the discussion about the spine. It was the idea that chronic stress may have a much greater effect on our physical health than many of us realize.
We spend a lot of time talking about nutrition, exercise, sleep, supplements, and the latest longevity treatments and devices. Those things absolutely matter. But what if one of the greatest influences on our long-term health is something we rarely treat with the same seriousness?
What if it is how much time our nervous system spends in survival mode? We readily accept that smoking, excessive drinking, inactivity, and a poor diet can shorten our lives. Maybe we should be viewing prolonged, unresolved stress with similar concern. We do not currently have evidence that allows us to say chronic stress is exactly as dangerous as smoking. But we do know it affects nearly every major system in the body, including the cardiovascular, metabolic, digestive, immune, and nervous systems.
It is possible that someone can eat an excellent diet, exercise regularly, maintain a healthy weight, and still have an important health risk that is largely invisible: a body that rarely feels safe enough to recover.
Over the years, I have noticed something that has caught my attention. Some people seem to do everything right. They eat well, remain very active, maintain a healthy weight, and make their health a priority. Yet they react severely to viruses, struggle with chronic pain, or develop inflammatory and autoimmune conditions. Meanwhile, I have known others with far less healthy lifestyles who seem to remain surprisingly healthy.
Obviously, that is only an observation. Every person’s health story is different, and genetics, environment, previous illnesses, exposures, aging, and plain old luck all play important roles. But it raises an interesting question.
Could chronic stress be causing dis-ease, or making existing health problems significantly worse?
Stress is not the enemy. We could not survive without it. When we are faced with danger, we enter fight-or-flight mode, and the brain signals the release of hormones such as adrenaline and cortisol. Our heart beats faster, blood pressure rises, stored energy becomes available, and our focus sharpens.
It is an incredible survival system. It protected our ancestors from immediate danger and continues to protect us today. The problem is that this response was designed primarily for temporary threats, followed by a period of recovery.
Modern stress looks very different. Deadlines. Financial concerns. Caring for aging parents. Raising children. Relationship problems. Health worries. Constant notifications. Nonstop news cycles. Difficult work environments. Uncertainty about the future.
None of these requires us to run from a predator, but our nervous system may react as though we are facing one. When these pressures continue day after day, the body can remain constantly flooded with stress hormones and inflammatory chemicals.
Our bodies were built to experience stress, recover, and return to balance. Many of us experience the stress but rarely complete the recovery.
Researchers have found that chronic stress does not simply affect how we feel emotionally. It can influence sleep, blood pressure, blood sugar, digestion, memory, immune function, pain, and inflammation throughout the body.
One well-known series of studies from Carnegie Mellon University exposed healthy volunteers to common cold viruses. Researchers found that people experiencing greater levels of prolonged stress were more likely to develop illness after exposure than those reporting less stress.
Scientists have also found that chronic stress can interfere with the body’s ability to regulate inflammation. Under normal circumstances, cortisol helps control the inflammatory response. But when the body is exposed to stress hormones repeatedly, immune cells may become less responsive to cortisol’s signals.
The result may be an immune system that has a harder time responding appropriately. It may become less effective at fighting certain infections while also allowing inflammatory activity to continue longer than it should.
Inflammation itself is not necessarily bad. It is an essential part of the body’s healing process and plays an important role in fighting infection and repairing injuries. The concern is what happens when the inflammatory response does not turn off when its work is finished.
We often think about health risks as things we can see or measure: cigarettes smoked, alcoholic drinks consumed, pounds gained, blood pressure readings, cholesterol levels, or minutes of exercise.
Stress is harder to quantify. That may be one reason we tend to underestimate it. No responsible physician or researcher would say that every stressful period is equivalent to smoking. Stress is also highly individual. The same experience may be overwhelming to one person and manageable to another, depending on duration, circumstances, support, previous experiences, and the opportunity to recover.
Still, some findings suggest that we should take psychosocial health much more seriously than we do. For example, researchers studying social isolation and loneliness, both of which can become powerful sources of chronic stress, have found associations with premature mortality comparable to smoking up to 15 cigarettes a day. That statistic does not mean ordinary daily stress is equal to smoking. It does demonstrate that conditions we sometimes dismiss as merely emotional can have consequences that are as physical and significant as those associated with familiar lifestyle risks.
We may look back someday and wonder why we spent so much time perfecting our diets and exercise routines while accepting constant nervous-system overload as an unavoidable part of life. Perhaps chronic stress is not a secondary wellness concern. Perhaps it belongs alongside sleep, movement, nutrition, smoking, alcohol use, and social connection as one of the central factors influencing long-term health.
Back in Control examines why chronic pain can continue even when physical findings do not fully explain its severity. Sometimes the nervous system becomes overly protective, continuing to sound an alarm long after an injury has healed or the original threat has passed.
The pain is real. What has changed is the sensitivity of the alarm system. This same nervous-system response may affect much more than pain. When the body continually perceives danger, it may have a harder time directing its resources toward digestion, repair, restorative sleep, immune balance, and healing.
Chronic stress has been associated with higher inflammatory markers, slower wound healing, poorer sleep, increased susceptibility to some infections, cardiovascular disease, metabolic dysfunction, anxiety, depression, and flare-ups of several inflammatory conditions.
This does not mean stress directly causes every one of these diseases. Most illnesses develop through a combination of genetics, environment, lifestyle, aging, previous exposures, and other factors. But chronic stress appears capable of making many health problems more difficult for the body to manage.
That distinction matters. Stress does not need to be the sole cause of a disease to have a meaningful effect on its development, severity, or progression.
Researchers use the term “allostatic load” to describe the cumulative effects of repeated or prolonged stress on the body. It is the wear and tear that can build when the stress response is activated again and again without sufficient time to recover.
Unlike weight, muscle tone, or fitness, it is not something we can easily see in the mirror. A person can appear healthy, exercise regularly, and have relatively normal routine laboratory results while still carrying a substantial physiological burden from years of poor sleep, caregiving, financial pressure, unresolved trauma, work stress, loneliness, conflict, or persistent worry.
Over time, that accumulated burden may affect blood pressure, blood sugar regulation, immune function, inflammation, cardiovascular health, and the body’s ability to recover from illness or injury.
We would never tell someone that smoking is harmless simply because their laboratory results are currently normal. Perhaps we should be equally cautious about assuming that prolonged stress is harmless simply because its effects are not yet obvious.
Longevity may depend not only on how well we challenge and nourish the body, but also on how often we give it a genuine opportunity to recover.
Reducing chronic stress cannot remove every health risk. It will not replace medical care, a nutritious diet, exercise, or appropriate treatment. But it may help the body regulate inflammation, support immune function, sleep more deeply, and respond more effectively when illness or injury occurs.
The encouraging news is that the nervous system is adaptable. Regular exercise, restorative sleep, meaningful relationships, time in nature, mindfulness practices, slow breathing, and even a daily walk may help regulate the stress response. None of these eliminates life’s challenges, but they can help the body move out of survival mode and return to a state where repair and recovery are possible.
The goal is not to create a perfectly stress-free life. That is neither realistic nor necessary. The goal is to stop allowing stress to become a permanent physiological state.
Many people are willing to invest considerable time and money in supplements, testing, specialized diets, fitness programs, and longevity treatments. Yet we may hesitate to make changes to a chronically stressful schedule, leave a damaging situation, seek help for unresolved emotional pain, prioritize sleep, or create uninterrupted time for recovery.
Those choices may not feel as concrete as taking a supplement or completing a workout, but they may be every bit as important. Back in Control focuses on chronic back pain, but its broader message is relevant to anyone interested in health and longevity. Chronic stress affects pain, inflammation, immune function, sleep, and recovery. That makes stress management much more than a matter of simply feeling calmer.
We may not yet be able to calculate the exact number of years chronic stress takes from a life, or compare it precisely with cigarettes, alcohol, or poor nutrition. But the absence of a simple number does not mean the risk is insignificant.
We already know enough to stop treating chronic stress as harmless. We may not be able to remove stress from our lives, but we can give the body more opportunities to recover from it. Consistent sleep, time outdoors, regular movement, meaningful social connection, quiet time without a phone, slow breathing, meditation, and addressing the sources of ongoing stress can all help interrupt the cycle.
None of these requires a complete lifestyle overhaul. Even a daily walk, a few quiet minutes, setting one necessary boundary, or taking time to connect with someone can signal to the nervous system that the immediate threat has passed.
Over time, those small moments of recovery may be doing much more than helping us feel better. They may be protecting our health and, quite possibly, extending our lives.
